Put a newsletter signup-form on my squeezepage layout

Hi, This is a very simple job. I already have a HTML squeezepage created, but I need a form for people to sign-up to a newsletter laid on top. Take a look at the squeezepage here: [login to view URL] This is the form I need added on top: [login to view URL] The form must look EXACTLY like this, with fonts/boxes/shadow/everything! The form must be EASILY configured with BOTH aweber and Mailchimp. "Here goes a headline" and "see exactly how......." must be html-editable as textformat. - Not sure how you're going to get the red underline under a specific word, maybe with CSS? I have everything ready from HTML+CSS files for the squeezepage itself, all you need is to add the box as you see displayed on the picture. AGAIN - It needs to look 100% like it, not 90% with a different font and other boxdesign or sizes. Need this done TODAY. Not someone who says they can do it "immediately" but then it gets delayed for days and days. Need it today. Small job for someone who knows what they're doing, bid accordingly and don't send me some standard reply text with all your references and what you can do and blablabla. Tell me if you can do it and ill hire you. If you plan on bidding more than $30-60 then just find another project to bid on as this job is very small and should not take more than 1-3 hours for someone who knows what they're doing.
