Dear Employer ! Not sure you know this or not BUT: Even when you're on a PLUS plan, you CANNOT add a custom page to the checkout page to collect additional data. This can be done with a trick. The drawback is that: This method IS NOT supported and recommended by Shopify. It works for a few years now but THERE'S NO WARRANTY that it still works in the future.
