What is the sql server query to list the first 4 appointments of the patient and display the results in a column showing the date in columns 1 , 2, 3, 4, Patient Name Appt 1 Appt 2 Appt 3 Appt 4 Alice mm, dd, yyyy etc. etc. etc. Bob etc. etc. etc. etc. Peter etc. etc. etc. etc. Format the date in mm/dd/yyyy without the mintues and seconds.
